Abstract

The Community Solutions for Health Equity program (CSHE) is a grant-making and technical support program funded by the Robert Wood Johnson Foundation (RWJF). In this final evaluation report we discuss the multi-year evaluation activities, the CSHE theory of change and how it maps to its values, the outcomes of the program, and discussions and recommendations for the future.
